Athlete Eligibility Check List

Please come into the ASB Office and pick up all required paperwork.  All components of the athletic packet must be completed prior to turning it in.  Return completed packet to the ASB Office. Athletes WILL NOT be able to tryout or participate until the following guidelines are sufficiently met.  For questions call 885-6561. 

Prior to first practice and or tryout

ð     A valid sports physical on file in the ASB Office.  Please make arrangements for a sports physical well in advance of your sports season.

ð    Medical insurance through the parent or the purchase of school sponsored insurance.

ð    Parental permission signature on both sides of the permission form and Athletic Code agreement.

ð     Athlete's signature for knowledge of safety rules and conduct code.

ð     Athlete and parent signature on the concussion awareness form.

ð    Completed “Emergency Treatment Authorization” form.

Prior to first competition

ð    All fines must be cleared before participation.

ð    Meet BGHS academic and residential requirements.  All non-BGHS students (CAM, Homelink, Online Academy, etc.)  need to bring a copy of their grades to the ASB office.

ð    Athlete must purchase a valid ASB card ($35)

ð    Paid participation fee. For information regarding participation fee costs, click here.  For “cut” sports, DO NOT pay participation fee until all cuts have been made.
